Health Home Care Management
Health Home Care Management helps eligible Medicaid members with complex needs navigate medical care, behavioral health services, and community supports. ASCEND is preparing to deliver these core care management functions with professionalism, accountability, and a person-centered focus.
Care Coordination
Helping coordinate communication between physicians, specialists, behavioral health providers, hospitals, pharmacies, and community organizations.
Transitional Care
Supporting individuals during transitions following hospitalization or emergency department visits to reduce complications and improve continuity of care.
Community Resource Navigation
Connecting individuals with housing resources, food assistance, transportation, financial benefits, and other community supports.
Health Promotion
Helping members better understand their health conditions while encouraging preventive care and healthy lifestyle choices.
Family & Caregiver Support
Partnering with family members and caregivers to promote coordinated, person-centered care.
Preparing to Serve
ASCEND is preparing to provide Health Home Care Management services through partnerships with New York State Lead Health Home organizations.
